Paying by credit card?
Read this first to avoid delays in processing your payment!
In order to process your credit card for
payment, you MUST provide us with the following
information:

Billing address
of credit card holder (address where credit card bill
is sent)

City, State &
Zip code

The Credit
Card Number to be charged
- The Expiration Date
of the card

CVC Number
(MANDATORY) - This is the 3-4 digit code on the
back of your credit card. We CANNOT process your request
without it.

Your Signature
(anywhere on the registration card or form). This gives
us your authority to charge your credit card.
If we do not have ALL of the above
information, we can not charge your credit card! If you have
any questions or concerns, please contact Lisa at our Administrative
